• Powered Industrial Trucks - Inadequate worker training and refresher training.
• Machine Guarding - Exposure to points of operation.
• Fall Protection - training requirements - Failure to train workers in identifying fall hazards and proper use of fall protection equipment.
• Electrical - wiring methods.

The violations listed above are particularly common on construction sites where dangerous machinery, materials, and heights are commonplace.
